This Notice of Privacy Practices describes how medical information about you may be used and disclosed by Little Guyana Pharmacy and how you can access this information. Please review this notice carefully.

Little Guyana Pharmacy is committed to protecting the privacy of your health information. We are required by law to maintain the privacy and security of your protected health information and to provide you with this notice of our legal duties and privacy practices. We must follow the duties and practices described in this notice and provide you with a copy upon request.

For treatment purposes, we may use or share your health information to fill prescriptions, communicate with your healthcare providers, and ensure appropriate medication therapy. Any changes will apply to all protected health information we maintain and will be made available upon request and posted on our website. The effective date of the notice will be updated accordingly.

If you believe your privacy rights have been violated, you may file a complaint with Little Guyana Pharmacy or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
